Memory read operation timed out

I'm running CrossWorks 2.3.1.  The target is an STM32.   On specific boards, I am getting this error consistently.  On others, I am not (both running the same firmware).  It is possible that this is related to NVM contents because the boards displaying this problem were off the shelf.  Or, it could be some other hardware issue.

 

Is there any more explanation on what this error indicates and why the debugger disconnects?
